End Time Worship - Request Form

Thank you for your interest in our ministry. We hope to make our time with you as fruitful and lasting as possible. Therefore, we ask that you consider the following basic guidelines to accompany your request for ministry with End Time Worship. After reading the following, please complete the form below, which allows us to better evaluate your needs and prepare for your request. 1.) We consider a committed prayer covering before, during and after our ministry with you to be a necessary component for lasting effect on your church and community. 2.) We request clean, non-smoking (due to the health of one member) accommodations for team members in a motel/hotel/ or family home (unless otherwise agreed upon). 3.) We ask that a book table be available for our resource materials before and after the meeting, along with a local volunteer to assist with distribution (so that we can be free to adequately minister to people). 4.) The only financial commitment we ask is that our travel expenses be covered in advance, along with accommodations and local travel to be provided by the hosts. We do not mind being driven around by people in the church and do not require a rental vehichle. Hotels are not required either, but we would like to discuss arrangements before we arrive. We do not require a specific honorarium. We do ask that love offerings be received for our team each meeting (unless otherwise agreed upon). 5.) ETW likes to minister with local church leaders during our stay, but we request that no other engagements or personal meetings be scheduled without prior approval by End Time Worship (so that we can adequately keep our word given to the places that are hosting us). We are though flexible and are willing to be lead by the Holy Spirit should He lead us. We also encourage the ministry/church that is bringing us in to host an outreach ministry time where we can evangelize in word and song. Let's take the church to the streets. 6.) Lastly we request adequate water to be provided for each meeting, outreach or conference for each member of ETW (due to the fact the we do not always have access to water or the means to get around in your area). Thanks again for your interest in our ministry. We appreciate your respect in regard to the parameters mentioned above. These are some guidelines, but they are certainly negotiable and open for change if need be as we dialog during this process. We give prayerful consideration to all requests for ministry. However, due to ETW's ministry responsibilities and family commitment, we must sometimes restrict the types of engagements we can accept.You will be notified of our availability shortly after we receive your request for ministry.
